So let’s talk about something I hear almost every single day in the office. Someone comes in and they say, Dr. Tiss-dhal, my back was doing great for a while, but now it’s back again. Why does this keep happening to me?

And honestly, that’s one of the most frustrating things about lower back pain, right? It seems to have a mind of its own. You feel better, you get back to normal life, and then bam… it shows up again out of nowhere.

Here’s the thing, though. It’s not actually out of nowhere. And once you understand why it keeps coming back, you can actually do something about it.

The first thing you need to know is this. Most people treat the pain, but they never fix the problem that caused the pain in the first place. So let’s say your back hurts because you’ve been sitting at a desk all day with terrible posture. You ice it, maybe take some over the counter medicine, and the pain goes away. Great.

But did anything actually change? Are you still sitting at that desk the same way? Are the muscles still weak? Are the joints still stiff? If the answer is yes, then of course the pain is going to come back. You put a band-aid on it, but the underlying issue is still there waiting for the right moment to flare up again.

Now, the second reason lower back pain keeps returning is something we see all the time… and that’s incomplete healing. You start feeling seventy or eighty percent better, and you think, okay, I’m good. So you jump right back into your normal routine. You start lifting heavy things, playing sports, doing yard work, whatever it is. But your body wasn’t actually ready for that yet.

Think of it like a sprained ankle. Just because you can walk on it doesn’t mean it’s fully healed. The same goes for your spine. The pain might be gone, but the soft tissues, the muscles, the ligaments… they need time to rebuild strength and stability. If you don’t give them that time, you’re just setting yourself up for round two.

And here’s the third piece of the puzzle. Your body has something called muscle memory and movement patterns. If you’ve been moving incorrectly for months or even years, your brain has learned that pattern. So even after we get the joints moving better and reduce the inflammation, your body might still default back to those old faulty movement habits. That’s why rehabilitation and corrective exercises are so important. We’ve got to retrain your nervous system to move the right way.

So what can you actually do about this? First, when your back starts feeling better, don’t just stop there. That’s the time to build on that progress. Strengthen your core. Stretch regularly. Keep moving in healthy ways. Your spine needs support from strong muscles around it.

Second, pay attention to your daily habits. How are you sitting? How are you bending and lifting? How’s your workspace set up? Those little things add up big time over weeks and months.

And third, be consistent. I know life gets busy. But if you only stretch when your back hurts, or you only think about posture once in a while, you’re always going to be chasing your tail.
